Kindred Transitional Care and Rehabilitation - Greenfield, located in Greenfield, Indiana near Indianapolis, provides nursing, pharmacy, dietary and social services, as well as specialty programs in orthopedics, stroke recovery, pulmonary and Alzheimer's care. We are improving our clinical capabilities and staffing in order to accept patients with greater medical complexity than a traditional nursing home. This is increasingly important as many higher acuity patients require transitional care before they are ready to return home. As a Transitional Care Center, we strive to assist our patients and residents as they transition to the next level of care, whether that is home, assisted living or long-term care.

Our center is in Greenfield, Indianapolis, near five major hospitals and has been recognized for our continued dedication to outstanding service by receiving the Bronze - Commitment to Quality award from the American Health Care Association.
